Mr Maxwell has returned to clinic after a 3 month period of supervised exercise, which he has tried his best to attend. He has stopped smoking. He is very limited by his claudication because he has to keep stopping at work because of the pain. He has managed to lose 1Kg but his walking distance is worse than before and he can now only walk 50 yards.

• His blood pressure is 145/90 and his ABPI is reported as 0.68 on the right leg and >1 on the left leg.
• His exercise ABPI shows a drop to 0.5 on the right leg but shows no drop on the left side
  
• His ECG shows some T wave changes.
  
• His CXR is normal
