Further History

He has been a heavy smoker for 50 years (2 packs of cigarettes/day) and he had a chronic smoker's cough for "10 or 15 years". However, his cough was mild, non-productive and occurred most often in the early morning.

He has lost a lot of weight recently, about 10 kg over the last 3-6 months. His appetite is good and he does not complain of any change in his bowel habits.  He has a history of hypertension, hyperlipidaemia, COPD.

He is taking

  • Lisinopril 10 mg
  • Atorvastatin 20 mg nocte
  • Tiotropium 18 mcg
  • Fluticasone/salmeterol inhalers

He drinks about 28 units of alcohol daily. He is divorced and has 3 (2 sons, 1 daughter)  children age 50, 45 and  40 years.

There is a family history of hypertension and cardiovascular disease.
