  • He has a history of high blood pressure. This has been treated by his GP for the past 10 years and readings obtained on regular review have been in the region of 150-156 / 84-96.
  • There is a family history of stroke and heart attacks in relatives aged 50-70.
  • He is a life-long smoker since the age of 16-20 cigarettes per day. He has recently been attending a smoking cessation clinic; this was arranged by his GP. Unfortunately he continued to smoke and found the nicotine patches unhelpful.
  • He has never had his cholesterol checked.
  • He is not known to have diabetes.
  • He has never been to hospital before.
  • He drinks on average ½ bottle of wine per day and has done for many years.
  • He lives with his wife in a 2 bedroom flat, which they own and is on the top floor of a converted Victorian terraced house. There are 40 steps to climb to reach the bedroom and bathroom.
  • He and his wife have 2 children who live abroad.

On direct questioning, he has no history of palpitation, chest pain or breathlessness and there is no history of previous TIA. He has no headache now or before.
