General Examination

Observations

  • Alert, unable to speak, overweight (BMI 32).
  • Not pale or sweaty.
  • Examined lying on a hospital trolley.

 

Cardiovascular System

  • Pulse 90 regular.
  • BP 172/105.
  • Peripheral pulses present.
  • No carotid bruit.
  • Heart sounds I+II, no added sounds.

Respiratory System

  • No abnormality detected

Gastrointestinal System

  • No abnormality detected

Genitourinary System

  • No abnormality detected

Mental/psychiatric exam

  • Examination not performed

Musculoskeletal system

  • No abnormality detected

Nervous system

Dysphasic. Is able to understand speech but is unable to complete a sentence (expressive dysphasia). Cannot name simple objects such as pen, tie, book.

Cranial nerves
  • I – normal
  • II – PEARL, right homonymous hemianopia, Grade II hypertensive changes in fundi
  • III, IV, VI – slight gaze deviation to the left
  • V – appears intact
  • VII – right UMN palsy
  • VIII – normal
  • IX-XII – good palatal elevation without deviation and good tongue movements
Limb examination

Tone is decreased in a flaccid fashion in the right arm and to a lesser extend the right leg. Power graded according to the MRC grade (see below)
