General Examination

  • Looks unwell, pale, clammy
  • P105, BP 70/40, Oxygen Saturation of 92% on air
  • Temperature normal
  • Corneal arcus and xanthelasma noted

 

Cardiovascular System

  • Cold and clammy hands with tendon xanthomata over knuckles
  • Pulse 105/min, regular low volume
  • Blood pressure 70/40
  • Bilateral xanthelasma and corneal arcus
  • Heart sounds normal
  • Peripheral pulses absent

Respiratory System

  • Trachea central
  • Expansion: normal and symmetrical
  • Percussion: normal
  • Tactile vocal fremitus: normal
  • Breath sounds: fine late inspiratory crackles bilaterally from lower to mid zones

Gastrointestinal System

  • Tender and guarding over epigastrium, left side of abdomen and left flank with no peritonism
  • Pulsatile central mass i.e. AAA
  • Liver, spleen, kidneys not palpable
  • Hernial orifices normal
  • Bowel sounds normal
  • Rectal and pelvic examination not performed
