Adam is suspected to have hypertrophic cardiomyopathy with obstruction on the basis of his symptoms and presentation, the ECG appearances, and his family history. He is admitted for 24 hours during which time he undergoes an echocardiogram which confirms the diagnosis and a 24-hour Holter monitor and exercise tolerance test, the results of which are given above. He is commenced on a beta-blocker, bisoprolol 2.5 mg once a day, with instructions to his GP to up-titrate the dose to 5mg or more.
Adam feels well the next day and is discharged home with an outpatient appointment for a cardiac MRI scan and follow-up in clinic with the result.
